Access to RCA Resources Portal, one-time in-person assessment ($300 and includes initial review, of family history, review of medical records, review of medication list, home safety assessment/recommendations, and detailed care plan outlining goals), Coordination of Services, Emergency Plan, monthly virtual caregiver support groups, and all of the benefits listed below.
